We work directly with self-managed, plan-managed, and NDIA-managed participants.
No GP referral is required – you or your support coordinator can refer directly.

Complete our online referral form or email us with your details. We accept referrals from participants, carers, support coordinators, GPs, and other healthcare providers.

2. We Get In Touch

Our team will review your referral, call you to confirm details within 1-2 business days, and discuss the best service options for your goals and funding type.

3. Book Your First Session

Choose a convenient time for your initial assessment – at our Minchinbury clinic/gym, at home, via telehealth, or in your community.
